To set up multiple account overdraft protection, to authorize us to pay one-time ATM and Debit Card transactions, or to opt-out of courtesy pay please submit your request by mail (PO Box 5286, Atlanta GA 31107), fax (404-577-8529), bring it in, drop it in the night drop, or email it to us at bondcu@mindspring.com.

- Member Number
- First priority overdraft protection member number and suffix
- Second priority overdraft protection member number and suffix (if applicable)
- Third priority overdraft protection member number and suffix (if applicable)
- Fourth priority overdraft protection member number and suffix (if applicable)
- Fifth priority overdraft protection member number and suffix (if applicable)
- Written Autorization to revoke your courtesy pay benefits (if applicable)
- Written Authorization to add Courtesy Pay Services to pay one-time ATM and Debit Card transactions to your account
- Primary member’s name
- Joint owner’s name (if applicable)
- Member and Joint owner’s Signature (Mandatory)
- Date
If you want to email this form to us:
- Download the form and save it to your computer
- Fill it out the form in its entirety
- In the Signature line, type your name in lieu of your signature
